Level 4 Employee
As a level 4 Employee you will be scheduled 5-days a week. Your schedule will vary week to week depending our client’s needs. You will be on-call 1 of the 2-days that you are not scheduled. You will be guarantied a minimum of 30-hours per week and you will not be asked to work more than 55-hours/week. To be paid for 30-hours you must work all the hours you are assigned including any on call hours assigned to you. If the reason you have not worked 30-hours was because you were not available, you will only be paid for the hours you actual work. You will also be guaranteed one-weekend off every 6-weeks.

Example 1)
If you worked 40 hours and have earned your bonus
Your paycheck for that week would be calculated as follows:
40 hours x 11.00 = 440.00
Bonus 40-hours x 2.50 100.00
10.00 Gasoline Money 10.00
Total Pay $550.00
Example 2)
You work 49 hours and have earned your bonus
Your paycheck for that week would be calculated as follows:
40 hours x 11.00 440.00
9 hours x 16.50 (time and a half) 148.50
$10.00 Gasoline Money 10.00
Bonus 40 x 2.50 120.00
Total Pay 718.50
We expect level 4 employees to earn between $30,000 and $35,000.00 per year

Duties
- Covering for caregivers when they call out. (Expect last minute calls)
- Setting up new clients
- Working with our existing client base
- Performing Spot check at client’s home
- Training new hires
- You must be available from 7am to 10pm including weekends and occasionally overnight shifts from 5pm to 8am shifts. You will never be scheduled for more than 55-hours in any one week.
Compensation
- Base rate 11.00 per hour plus a bonus of *$2.50 per hour.
- You will also be paid 10.00 per week as a gasoline allowance.
- Any hours worked over 40 will be paid at time-and-a-half the 2.50 per hour bonus, however, will not be paid on hours worked over 40 as those hours already are being paid at time-and-a-half.
- Benefits will be available after 30-days
- 2-day paid day off after 12-months

- You will lose your entire bonus for the pay period (pay period are two weeks long) if during the pay period one of the following occurs.
- You refused or cannot cover a shift.
- You cannot be reached or refuse to cover a shift when needed except on your Guarantied Day Off
- If you are unavailable for work
- If you are terminated
- You are a no-show to a scheduled shift
- You send a text message about not being able to work rather than calling.

Below, is some typical situation that will arise and what we will expect from you is described below. Keep in mind you will not be asked to work more than 55-hours per week.
- A caregiver calls out at 6:30am because she is sick her shift was from 8am to 12pm. You are scheduled to work with another client from 1:30p to 4:30p. We will expect you to cover the 8am shift and then proceed to your 1:30pm shift.
- On your on-call day-off, a caregiver during her 8am to 4pm shift becomes ill and cannot finish her shift. We will expect you to promptly relieve her and finish the shift..
- You are scheduled to work with a client M-F from 8a to 4p On Sunday night we bring a new client on-board who immediately needs 24-hour care. To cover that client we change your schedule, canceling your 8am to 4pm hours on Monday and Tuesday and schedule you to work with our new client from Monday night from 5pm to 8am.
